The Integrated Mental Health Therapist (IMHT) works within a multidisciplinary team embedded in the primary care clinic and utilizes the Collaborative Care model of care. This team includes the IMHT, Psychiatric Consultant, & Primary Care Provider to provide comprehensive behavioral health services within the primary care setting. IMHT will provide evidence-based interventions, in partnership with the treatment team, to promote symptom reduction and optimal patient outcomes. Responsibilities: -Assesses patient’s symptoms and create customized treatment plan based on individual patient needs -Screens patients for common behavioral health disorders using PHQ9, GAD7, Columbia Suicide Risk Assessment, or other tools as required. -Monitors patients for changes in clinical symptoms and tre...
