Delivers services in accordance with the approved plan of treatment and plan of care. Assists behavioral healthcare providers in understanding significant social and emotional factors related to the patient's behavioral health issues. Education: - Identifies client’s behavioral health needs, plans for implementation and/or coordinates care plans through resource acquisition, monitoring, advocacy, collaboration, and evaluation. (20%) - Coordinates follow-up treatment referrals to various levels of care including outpatient therapy and psychiatry, intensive outpatient programing, long term care and other services as indicated.
