This is a highly professional position with lead responsibilities for linking and re-engaging Human Immunodeficiency Virus (HIV) positive clients to care in the 7 counties of Area 8. This position will work with the current Human Immunodeficiency Virus/Acquired Immune Deficiency Syndromes (HIV/AIDS) and Sexually Transmitted Disease (STD) intervention staff to identify clients who are not in care. This position will work with the data to care team (D2C) from State health office, as well as with community partners in the identification of those out-of-care clients and those clients who have been released from the county jail.
