Employment Type: Part time Shift: Weekend Shift Description: Job Title: RN Case Manager Employment Type: PRN: Shift: Weekend Day Shift - 8-hour shifts (Must pick up at least 3 shifts a month) Location: Holy Cross Hospital - Silver Spring Position Purpose: As a Case Manager focused on discharge planning, you will play a vital role in guiding patients through a seamless transition from hospital to home or post-acute care. By developing tailored discharge plans, educating patients and families, and coordinating access to resources, you will help ensure a smooth recovery process. Collaborating with a compassionate and skilled interdisciplinary team, you’ll make a meaningful impact by enhancing patient outcomes and reducing readmissions. As a RN Case Manager, you will: Develop and implement patient-specific discharge plans to ensure safe transitions from hospital to home or other care settings.
