POSITION LOCATED IN NEW BEDFORD: JOB SUMMARY: Under the supervision of their assigned supervisor, the Caseworker is responsible for ensuring that all clients receive professional, safe care in a well maintained and secure environment; limit setting, behavior management, family work, the teaching of daily living skills, facilitation of medical/dental appointment and follow-up, school tracking, and facilitate psycho-educational groups. The Caseworker acts as a liaison with the client’s family, Department of Children & Families (DCF) Social Worker, school placements, and medical/dental practices to ensure that families and providers understand clients’ needs and that basic needs are being met. JOB FUNCTION: Program/Department Responsibility: - Advocate for clients in their home, school, and the community according to the individual treatment plan on a daily basis. - Identify community providers and medical and dental needs of clients following intake. - Schedule appointments as needed with medical/dental providers. - Seek out medication orders/PRN protocols forms/documentation of immunizations and most recent routine appointments for each client. - Facilitate medical/dental/eye appoin...
