The Social Worker will work alongside the case managers and housing specialists to engage participants in need of permanent supportive housing. This includes supporting the program modalities and individual services. Essential Duties and Responsibilities: Provide support to the housing specialist, case managers to ensure housing applications are completed timely. This includes the timely completion of psychosocial assessments for CAPS applications. Review incoming referrals and implement treatment plan recommendations for the case management team.
