CPCHG covers inpatient, outpatient and hospice. Position Highlights: - This is a Hybrid role, which will include community palliative & hospice and mixed locations (patient homes, Acute, SNFs, and Office) - Geographic Focus: Mecklenburg County & Cabarrus County - Full-Time Schedule: Monday – Friday, 8AM – 5PM - Call Schedule: 1 in 4 (with APP taking 1st call) - Join 4 Physicians & 6 APPs - Strong referral base - Program hosts medical students, residents, APP and MD fellowships - Chaplain support is available Ideal Candidate: When you join Atrium Health, you will be welcomed into a cul...
