The purpose of the Regional Coordinator (RC) position is to serve as the region team lead and supports team members with care coordination consultation, regional case triage, resource development and both regional training related to resource development and supportive services best practices identify the behavioral health, rehabilitative, and other services needs of service members, veterans and/or their dependents/family members (SMVF). In addition to stay connected to direct services delivery and oversight the RC works with clients to develop a coordinated services plan, connect the individual with appropriate service providers, offer care coordination support/assistance, and monitor for the successful resolution of the identified needs.
