OLD COLONY Y JOB DESCRIPTION: TITLE: Case Manager, OCC Community Justice Support Center JOB CLASSIFICATION: Non-Exempt REPORTS TO: Director, OCC CJSC JOB SUMMARY: The Case Manager is responsible for the overall implementation, assessment, and administration of quality casework services at the Center. S/he will display and encourage the character traits of honesty, respect, caring and responsibility in the fulfillment of his/her job duties. JOB FUNCTIONS: Program/Department Responsibility:  Ensure that all casework services are consistent with the operating policies of the Office of Community Corrections (OCC) and other regulatory agencies.  Ensure that participant documents such as a copy of all intake materials are placed in participant files including those provided by supervising agencies (e.g., Probation, Parole, and Sheriff’s Department).  Complete intake assessments utilizing assessment tools (e.g. LS/RNR, TCUDS, etc.); assess risks/responsivity needs; and develop treatment plans for each assigned participant.  Ensure that treatment plans and goals for transition are completed and reviewed on a monthly basis.
