Mental Health Case Manager

Job description

The Blended Case Manager is responsible for developing, coordinating, and implementing services for consumers as identified in the service plan. Responsibilities include providing case management, service planning, and coordination of services with other agency providers to meet the client’s needs along with supportive counseling. The Blended Case Manager will also provide advocacy in order to help consumers set and achieve recovery goals and empower consumers to increase self-advocacy. Required Qualifications: - 12 credits in coursework related to Human Services, Psychology or related field - One or more years of direct clinical experience working with children, youth and families - Manage a varied work schedule Monday-Friday between 8:30am - 8:30pm, including 3 required evenings per week Preferred Qualifications: - Bachelor’s degree in Human Services or related field - Bilingual in Spanish.
