Job Description Registered Practical Nurse: Mission: Reporting to the Director of Care (DOC) or Director of Health and Wellness (DHW), the Registered Practical Nurse (RPN) is responsible for providing planned, patient‑oriented, goal‑directed nursing care in accordance with provincial standards of care. Part-Time: 30 hours biweekly, Days — 6:15 a.m. to 2:15 p.m. 30 hours biweekly, Evenings — 2:15 p.m. to 10:15 p.m. Temporary Part-Time (0.2 FTE): 1 day per week — 15 hours biweekly Key Responsibilities: Work as a member of the interprofessional team as described by Extendicare’s Professional Practice Model, applying a person‑centered approach to care. Complete and ensure resident clinical care assessments, care/service plans, and documentation are accurate, up to date, and communicated as required. Implement and evaluate the effectiveness of care/service plans and make changes as needed. Assist with scheduling resident care conferences and participate as applicable. Work closely with DOC/DHW to facilitate resident move‑ins, move‑outs, and transfers.
