Job Description Performs various aspects of hospital care within assigned patient care unit under direct supervision of a Registered Nurse. Job Responsibility: - Provides direct patient care as defined in the New York Nurse Practice Act in the Home, Nursing Facility or Inpatient or Residential Setting. - Applies hospice and nursing practice standards to patient care activities. - Implements plan of care initiated by the Hospice RN. - Provides accurate and timely documentation of patient services reflective of plan of care. - Reports changes in patient condition or needs to clinical staff. - Assess and provides patient and family/caregiver education and information. - Participates in coordination of hospice services, reporting needs for hospice interdisciplinary group members, including but not limited to: Hospice Aide, OT, PT, Speech, Nutrition, Skin, Bereavement Coordinator, Volunteer Coordinator or Clinical Supervisor referrals/assessments. - Provides appropriate pain/symptom management.
