Per Diem RN Case Manager – Homecare VNA of Care New England: School’s Out. Flexible Nursing Opportunities Are In. The VNA of Care New England is seeking compassionate and motivated Per Diem RN Case Managers to join our Homecare team. This role offers the flexibility to work around your summer schedule while providing one-on-one patient-centered care within the community. As an RN Home Health Clinician, you’ll manage a caseload of patients from admission through discharge while collaborating with a dedicated interdisciplinary team to support positive patient outcomes.
