C ompany: Oak Street Health: Title: Patient Engagement Specialist I: Location: Rhode Island area: Role Description: The Patient Engagement Specialist I will serve as an essential member of the Patient Engagement and Population Health Team. This role necessitates conducting patient outreach, encompassing telephonic communication and in-person home visits, to cultivate engagement within our care model. The PES I is responsible for engaging patients who have not yet visited the clinic or participated in our care model, providing support to facilitate their access to care at Oak Street.
