Job description

This position is partially or fully grant-funded. Employment beyond the term of the grant award may be subject to approval by Executive Council Here’s what you’ll be doing: - Serves as a central point of contact for clients requesting assistance from the Community Health Department and as a liaison between the Medical/Community Health staff, Family Services and other tribal programs on behalf of clients needing assistance. - Assesses client needs, including medical, emotional, psychosocial, and social determinants of health. - Develops individualized care or navigation plans and collaborates with interdisciplinary teams both within FCP and outside organizations and healthcare systems. - Facilitates communication between clients, families, internal and external healthcare providers, and other key resources. - Helps to identify and address barriers to care such as transportation, housing, insurance, appointment availability, financial challenges or other cultural barriers. - Documents all client visits into appropriate documentation systems following the FCPCHD documentation policies.
