POSITION SUMMARY: Under the supervision of the Program All-inclusive Care for the Elderly (PACE) PACE Medical Director, the PACE Clinical Pharmacist is responsible for a wide range of medication management services and processes provided through the PACE Center. The Clinical Pharmacist must be able to make evidence-based recommendations for optimizing drug therapy for our participants. The PACE Clinical Pharmacist will be responsible for: PACE Enrollee Comprehensive Medication Review, Transitions of Care Medication Reconciliation, Anticoagulation Management, Medication Compliance & Safety, CMS Fraud/Waste/Abuse, Part-D quality and safety compliance activities, drug utilization reviews, PBM (Prescription Benefits Manager) reporting and review, Acumen safety reporting & compliance, and PACE Interdisciplinary team (IDT) representation and participation.
