About the Role: We are seeking a passionate and experienced Chronic Disease Nurse to join our clinical team in Wellington. In this role, you will provide high-quality, culturally safe, patient-centred care to Aboriginal and Torres Strait Islander clients living with chronic conditions. You’ll work closely with GPs, Aboriginal Health Workers, and a multidisciplinary team to improve health outcomes and support long-term self-management.
