We're currently seeking a Licensed Practical Nurse (LPN) to join our residential care team! Job Summary: Provides direct care to individuals living in a residential setting. Follows the outlined plan of care, identifies health-related needs, monitors medical status, and reports changes to the on-call RN, and collaborates with residential staff to support the individuals in their community residence. Schedule: Sunday 3:00 pm - 11:00 pm, Monday 3:00 pm - 11:00 pm, Friday 7:00 am- 3:00 pm, Saturday 7:00 am - 3:00 pm (32hrs) Qualifications: Job Type: Part Time (32) hrs. Pay Rate: $42.00/hr. Work Environment: Works in the community in a home setting with either multiple residents or someone who lives independently. Health Monitoring & Care: - Conducts regular health assessments (on shift), including vital signs and symptom checks, to monitor individual health & determine nursing needs. Reports to and seeks direction from the RN Case Manager or on-call RN as needed. - Medication Management –Administers medications and treatments as prescribed and per MAP regulations.
